Cancellation Policy
The Mesquite Family Practice has instituted a formal policy regarding cancellations and “no shows.” A “no show” is defined as a scheduled appointment that the patient does not keep. Patients are expected to call to cancel if they cannot make a scheduled appointment.
No Show First occurrence: Patient will be sent a letter or called. No fine is assessed.
Second occurrence: Patient will be charged a $25 fee.
Any additional “no shows” the patient will be charged the full price of the scheduled office visit.
Email correspondence is accepted, but may not be secure. Urgent issues should not be communicated over email, but should be done in person or by phone. For emergencies, call 911. All email messages and responses will be posted in the patient's chart for documentation purposes.
After Hours
If a problem arises outside of our normal office hours and is an EMERGENCY(potential for loss of life, limb or vision), call 911. If you have another non-emergency situation that warrants immediate attention please call us at (972) 613 2127 and our messaging service will contact the on-call provider. You must be established with the practice for after hours assistance.
Medication refills
- Call your pharmacy first to request refills, even if your prescription says that there are no refills; the pharmacy will get in touch with us and we will get it completed.
- Refills of existing prescriptions will be filled within 2 business days of receipt of the request.
- You may need to see us prior to a full refill being given.
- Calls for medication refills will not be accepted outside of regular business hour Monday – Friday 9:30- 5pm.
Controlled Drugs
Refill of controlled drug prescriptions (narcotics, sedatives) will be limited to those originated at the family practice until we review copies of your prior physician's records. Formal, written, and signed medication plans may be required for ongoing controlled drug use. New patients - bring the old bottle or records of your prescription to your first appointment. Drug screenings will be completed randomly for patients on controlled medications.
Prescriptions
Your prescriptions may be faxed to the pharmacy of your choice at the time of your visit. Exceptions include medications that require a hand delivered script, or during evening and weekend hours when transmission cannot be verified. Please allow three full business days for prescription renewals.
Same-Day Appointments are available for acute problems.
Our board certified family physicians provide office based medical care for children and adults of all ages. Acute and chronic medical problems are handled by appointment only. Preventative medical services such as routine physical exams and immunizations are offered. Early detection programs for cancer, heart disease, and other common ailments are available
